Quizmakers freezes after the user passes the course

We use Syberworks as our LMS and it seems that I having trouble with some courses. Here's the situaion, the user passes the course and then shortly there after they try to launch the course again and receive this error :

Now this doesnt happen with every quiz that I have, it just seems to happen now and then to specific courses but not to others. Has anyone had this happen to them? I believe this error is on my LMS side and has nothing to do with quizmaker but I thought I'd ask the experts :-)

I used firebug for firefox to look at the error and this is what firefox reports :
